Urology & Urogynecology Surgery and Complications

Urology surgery involves a wide range of procedures on the kidneys, ureters, prostate, bladder, urethra, penis, or testis in men, addressing both benign and malignant conditions. This includes operations such as partial or radical nephrectomy for kidney tumors, pyeloplasty for ureteropelvic junction obstruction, ureteral reimplantation, prostatectomy for cancer or benign prostatic enlargement, cystectomy with urinary diversion, urethral reconstruction for strictures, penile prosthesis or curvature correction for erectile dysfunction or Peyronie’s disease, and orchiectomy or testicular-sparing surgery for tumors. Minimally invasive and robotic approaches are increasingly common in these areas.

Urogynecology surgery, by contrast, focuses on repair or reconstruction of the female pelvic floor for problems such as prolapsing organs or urinary incontinence. Procedures include sacrocolpopexy, hysteropexy, midurethral sling placement, colporrhaphy, paravaginal repair, and mesh or graft reconstructions. These operations may address cystocele, rectocele, uterine prolapse, stress urinary incontinence, and complications from previous pelvic surgery. Neuro-urology complications consist of urinary or sexual dysfunction that may result from nerve injury due to disc or spinal nerve compression, surgical nerve injury or patient positioning, stroke, traumatic brain injury, or other neurologic mechanisms. Common examples include spinal cord injury leading to urinary retention or erectile dysfunction, or pelvic trauma causing nerve damage to the bladder, urethra, or surrounding pelvic organs. “Vaginal rejuvenation” complications can include vaginal burns, scarring, painful intercourse, and chronic vaginal or pelvic pain, sometimes requiring additional reconstructive procedures.

Transgender surgery involves removing the penis, testes, uterus, ovaries and vagina, which sometimes reconstructing a neo-vagina, neo-phallus (penis), neo-scrotum. Implant or prosthetic surgery including penile implants, artificial sphincters, testicular implants, urethral stents, ureteral stents (JJ stents), InterStim, Axonics, sling mesh, colpopexy mesh, as well as removal or explantation of these devices.

Reasonable Review

Most surgery, if appropriately indicated and carefully executed, can achieve excellent outcomes and provide substantial relief of symptoms. However, complications may still arise at several stages of the surgical process, including during the technical execution of the procedure, improper patient positioning on the operating table, inadequate or rushed pre-operative planning, or errors in the immediate and long-term post-operative management.

Recognition of these complications and rapid intervention are essential to minimizing patient harm and preventing escalation of injury. Damage to the male or female genitals or urinary tract, whether caused by surgery, blunt or penetrating trauma such as motor vehicle accidents, or iatrogenic injury from device implantation and instrumentation, may lead to significant physical morbidity and psychological distress. Such injuries often result in chronic difficulties with urination, including obstruction, incontinence, or retention, as well as impairment of sexual function, ranging from erectile dysfunction to dyspareunia. Traumatic injury to the delicate neural pathways that govern urinary continence and sexual performance can further complicate recovery, as nerve damage may be irreversible or require extensive rehabilitation.

Careful assessment of the mechanism of injury, detailed evaluation of the structure and function of the genitourinary tract, and documentation of the clinical course are all critical for establishing medical causation, determining the extent of damages, and assessing the risk of long-term or lifelong disability. These evaluations also guide treatment strategies, medico-legal decisions, and patient counseling regarding prognosis.

What is a board certified urologist and board certified urogynecologist?

A physician trained in urology during a residency program and then passes a written exam and oral exam to certify competency in urology. After fellowship training in urogynecology, I took the board certification exam in female pelvic medicine and reconstructive surgery, also known as urogynecology. I recently re-certified for both in 2023. I am one of few urologists nationally who is double board certified in both specialties. 

When did you begin reviewing cases as an expert witness?

In 2013. I initially began reviewing cases during the pelvic mesh multidistrict litigation, but I simultaneously have been reviewing all types of urology and urogynecology cases as well. To date, I have reviewed over 400 cases, in nearly all the states, as well as Puerto Rico, federal cases, and Native Tribal lands. I have assisted attorneys for Plaintiffs, Defendants, health care systems, insurance companies, consultant firms, patent cases, criminal/sex crimes cases, personal injury, urology medical device injury cases, and transgender cases.

Do you review urologic oncology cases?

Yes, I am trained in and review cases involving prostate cancer, bladder cancer, kidney cancer (renal cancer), testis or testicle cancer, and penile cancer, including laparoscopic surgery.

Yes, I am trained in male reconstructive surgery from residency and female pelvic reconstructive surgery in fellowship.

It is a professional medical society founded in 1902 that represents urologists and allied health professionals. Its mission is to promote the highest standards of urological clinical care through education, research, and health policy advocacy. The AUA develops and publishes clinical practice guidelines, provides continuing medical education through courses and its annual meeting, sponsors research, and advocates for the interests of urology at the legislative and regulatory levels.

What is the American Board of Urology?

Founded in 1935, is a certifying body rather than a professional society. Its role is to define the training standards necessary to practice urology and to administer the examinations that grant board certification to qualified urologists. The ABU also oversees maintenance of certification, ensuring that practicing urologists remain up to date with evolving knowledge and clinical standards. The ABU does not provide membership services, advocacy, or education for its own sake, but rather serves as a credentialing authority to certify the competence of individual practitioners. That role is served by the American Urological Association.
